Overview
Under the direction and supervision of the Black Infant Health Program Director and with guidance and support from the Community Intensive Service Specialists, provides outreach and case management services to pregnant and parenting African American women.

Responsibilities
- Conducts community based outreach activities and networking to locate, reach out to and recruit African American pregnant and parenting women in need of BIH program services.
- Provides case management services and coordinates care as appropriate within and outside the FHCSD system.
- Develops and maintains community resource information related to pregnancy, parenting, and other social service needs relevant to African American women/children.
- Maintains period and regular contact with clients as defined by program guidelines.
- Participates in the development of program related materials and cirricula.
- In conjunction with other BIHP staff, plans celebratory activities for program participants and the community.
- Documents activities on relevant forms and referral logs.
- Assists MFT with group session as needed and appropriate.

Qualifications
- High school diploma or equivalent required although a college degree or some college courses in a social science or health related field preferred.
- Must have reliable transportation, valid driver's license and automobile insurance as required by law.
- Bilingual desired, bicultural sensitivity required.
- Familiarity with the particular community of service will be preferred.
- One to three years of experience in a community based or health care setting providing direct services to women and/or African Americans.
