Bachelor's Degree in a social science-related field. Preferred or three years related experience. ? Must possess valid California driver's license and appropriate auto insurance as required by law. ? Clinical education training (medical social worker or registered nurse.) ? Bilingual in Spanish/English preferred and sensitive to issues related to HIV and Latino populations. ? One to three years of work experience providing human services to high risk and/or disabled populations. ? One to three years of community service to persons with HIV/AIDS. ? Ability to maintain confidentiality. ? Excellent interpersonal skills. Responsibilities ? Promotes and manifests the ideals and goals of the health center. Provides the best possible care and services to any and all clients, patients, or visitors, with respect and courtesy, without prejudice or discrimination; and to strive for maximum client satisfaction in those encountered or served at the health center or while in its service. ? Participates in and supports the health center public relations and promotion and publicity directed at individuals and the community. ? Assists in the determination of linguistic and cultural appropriateness and efficacy of program materials, activities, methods, strategies and programs. Assists with the translation and/or revision of translated materials as appropriate or indicated. ? Participates in and contributes to project/program and other FHCSD services through participation in staff meetings and consultation sessions. ? Assist in preparing monthly, quarterly and annual reports to the corresponding supervisory or funding sources by submitting monthly reports in the appropriate formats of case numbers, meetings attended, training updates, networking updates and narratives of challenges, accomplishments and needs. ? Assists in developing appropriate case management procedures, forms and curricula, which are orderly, thorough and appropriate for multicultural client base. ? Responsible for initial and on-going assessment of client's health and/or support service needs by way of a face -to-face intake/interview with client, preferably on-site at FHCSD. Home visits may be necessary if client is too ill to travel. ? For clients ineligible for ongoing case management (asymptomatic with little need for ongoing support or care or already case managed elsewhere), is responsible for notification and referral to appropriate community resources. ? If client becomes symptomatic or wished to transfer service to FHCSD, then responsible for enrolling client into their case management program. ? Monthly client contact is required (phone contact is sufficient) and face-to-face contact is required quarterly. If client refuses contact, disenrollment from case management may be appropriate.
